As a parent, I acknowledge the privilege and responsibility I have to pass on the Catholic faith to my child. I will share and model my relationship with Jesus with my child by praying daily with my child, bringing my child to Mass every Sunday, participating regularly in the weekly program for which my child is registered, and by serving others.

If registering for sacrament preparation, I along with my child will attend each of the  seven Wednesday night prep sessions from 6:30-8:00 p.m. with my child. See  calendar for dates.  Please contact the Children's Faith Formation office if you need to arrange an alternate schedule  due to parents work schedule or custody arrangement only.

• have read the Parent Handbook and agree to all that it contains
• understand there will be forms that require my signature at the parent orientation or first day of classes.
